We are looking forward to continuing to work with your child as we prepare them for their special day!

WE ENCOURAGE YOU TO STAY IN YOUR CURRENT TRAINING DAY & TIME, if possible.

If you do not need to change your current day/time for the Fall you will be given the option to check a box below indicating that you would like to keep your current tutoring day and time.

IF YOU NEED TO CHANGE YOUR TRAINING DAY AND/OR TIME, PLEASE KEEP THE FOLLOWING IN MIND: We will do our absolute best to accommodate one of your top three choices which we will ask you to indicate below. We will also do our best to keep you with your current tutor.

Please understand that we cannot process your request form unless you provide us with THREE possible times as indicated in the guidelines above - should you need to change your time.

We have times available on Mondays,Tuesdays & Wednesdays 3-6 PM.  

We ask that you please fill out and submit this form no later than June 4th. Tutoring begins the week of September 15th, 2019.
