Thank you for taking the time to carefully read through, fill out and submit this form!

We would like to schedule the Bar/Bat Mitzvah service on or after your child’s birth date. Please help us know what is best for your family’s schedule by indicating your TOP THREE preferences on the form below. There is a space for you at the bottom to include any special needs or considerations. All attempts will be made to accommodate every congregant’s needs, but please remember this is a large group and some flexibility will be required. This is why we require you to select 3 choices in order of preference. 

Please note that the maximum seating capacity of the sanctuary is 350 people, or 175 per family. By indicating your best estimate of the number of guests you expect on the form, you will put us in a better position to minimize overcrowding.

Bar/Bat Mitzvah services are held on Friday evenings at 5:45 PM, Saturday mornings at 10:00 AM or on Saturday evenings at 5:00 PM. The services are similar and will likely be shared, except for the Friday evening service. The Saturday evening service incorporates a short Havdalah ceremony to mark the end of Shabbat.

Please return the form by May 18th, 2020. 

Please note that we will have an important family program at 10:00am on Sunday, June 7th, 2020 for parents and children called: “B’nai Mitzvah: The Journey Begins.” It will be an excellent opportunity to meet with clergy and other key players in the whole Bar/Bat Mitzvah process.
